Pachypodium bispinosum is propagated through seeds, which require high freshness to ensure a good germination rate. The ideal time for sowing is during the spring season when rising temperatures and longer daylight hours stimulate natural growth cycles.
A loose, mineral-rich substrate is essential for seed germination, typically composed of coarse sand and perlite. The seeds are placed on the surface and covered with a very thin layer of substrate to maintain moisture while allowing enough light for the sprouts.
Humidity must be kept constant using a transparent cover, while the temperature should be maintained between 22 and 25 degrees Celsius. Regular ventilation of the seed tray is crucial to prevent fungal growth, which is a common danger in high-humidity seed environments.

The substrate must be exceptionally well-draining, mirroring the rocky slopes of its natural habitat. A standard cactus mix enriched with additional pumice or grit is highly recommended to prevent any possibility of root suffocation or moisture buildup.
Watering should be approached with caution, following the rule of waiting for the soil to dry out completely before the next session. During the summer, waterings are regular but sparse, while in winter, the plant enters dormancy and requires almost total dryness.
Temperature control is essential for successful cultivation. While it can tolerate moderate heat during the growing season, it should be protected from frost and kept in a cool, dry environment during the dormant phase to promote spring blooming.

The primary threat to Pachypodium bispinosum is the development of rot due to excessive moisture or cold temperatures. If the caudex starts to soften, the affected tissue must be surgically removed, followed by the application of an antifungal treatment.
Pests such as spider mites and mealybugs are frequently encountered, especially in environments with poor air circulation or low humidity. These pests feed on the sap and can lead to the plant's rapid decline if left unchecked.
Chemical control involving systemic insecticides is effective against infestations. It is recommended to perform localized treatments while carefully following instructions to ensure the safety of the plant's sensitive tissues.
Regular visual inspections, particularly of the stem folds and leaf axils, are the best preventive strategy against mealybugs. Early detection allows for physical removal using a swab dipped in alcohol, which is often sufficient for minor pest occurrences.
